Primary Trusted Person Access Request

This page is only for the Primary Trusted Person (PTP) of an AfterPlan account whose owner has passed away or become incapacitated — and who cannot sign in through the normal flow (no invitation email arrived, or it was lost).

If you are the account owner and forgot your password, use Forgot your password? instead.

To grant you access as the designated PTP, we need to verify your identity and the owner's passing. Submit the documents below — we review every request personally and respond within 1–3 business days.

Your Email Address
Enter the email address registered with your AfterPlan access.
If you're not sure which email address was used, you'll need to contact us directly at [email protected] — please include a copy of the death certificate and a selfie holding your government-issued ID so we can verify your identity.
Your Full Name
Enter your full name as the account holder would recognize it.

Your photo must clearly show:

  • Your face
  • Your ID held under your chin in the same photo
  • The name and photo on the ID are readable

Accepted IDs: state ID, driver's license, or passport.
You may redact the ID number if you wish.
